      Nomination information:

      The Volunteer Activist Award is given annually to men and women who have made valuable contributions to their                         community.  These awards have been presented locally by the Baton Rouge Speech and Hearing Foundation for over 40     years.  Nominees for the Volunteer Activist Award must be from the Greater Baton Rouge area and be currently involved in volunteer activities for the betterment of the community.  Career or other professional accomplishments, or any activity for which the nominee received remuneration, will not qualify.

Please note that a past Volunteer Activist recipient may not be re-nominated for this award.  You may nominate more than one person; however, please do each on a separate form.  Also, please print or type the requested information regarding your nominee(s) and provide as much of that information as possible so that the Selection Committee can make informed decisions in conferring this honor.
